一、概述
为指导和规范辐照技术在中药灭菌中的正确应用,保证中药的安全、有效、质量稳定,特制定本指导原则。
本指导原则的辐照灭菌是指利用γ射线或是以电子加速器产生的高能电子束或转换成的X射线杀灭中药中微生物的过程,作为药品生产过程中降低药品微生物负载的一种手段。
本指导原则包括中药辐照灭菌基本原则及要求、辐照装置、辐照剂量和辐照检测等内容,适用于采用辐照灭菌的中药新药及灭菌方法变更为辐照灭菌技术的已上市中药。
二、基本原则及要求
(一)“必要、科学、合理”原则
因辐照灭菌应用于传统中药的灭菌历史尚短,基础研究需不断完善,故中药采用辐照灭菌应充分说明其必要性。申请人需要对产品研发和生产、产品性质等有全面和准确的了解。如采用辐照灭菌,应针对辐照灭菌对产品质量、稳定性、生物学性质等方面的影响进行全面研究和评估,通过提供的研究资料说明采用辐照灭菌的必要性、科学性和合理性。如处方药味含有结构不稳定成份的,应进行有针对性的研究,考察辐照灭菌前后成份不稳定成份的变化情况。
(二)“安全、有效、稳定”原则
中药采用辐照灭菌应以不影响原料或制剂的安全性、有效性及稳定性为原则。需要通过一定的研究工作考察和评估辐照灭菌对中药安全性、有效性及稳定性的影响。
1.应进行辐照前后的对比研究,包括采用指纹图谱等方法,尽可能全面地反映辐照灭菌前后药品所含成份种类或含量的变化情况。必要时,应采用与适应症相关的药效指标,比较辐照灭菌前后药品有效性的差异,或开展安全性研究。
2.对于毒性饮片或处方中含有毒性饮片的半成品,药材制剂的辐照灭菌,应关注辐照灭菌对药品安全性的影响。
3.凡灭菌工艺未被明确批准为辐照灭菌的已上市中药,若要采用辐照灭菌,应按《已上市中药变更研究技术指导原则一)》的相应要求进行研究。
(三)严格执行GMP的管理要求
辐照灭菌技术不能替代药品生产的GMP管理,中药药品生产过程中必须严格执行GMP规范,各个生产环节应设置降低微生物负载的措施,严格药材的挑选、清洁、炮制等加工环节,不应当将采用辐照灭菌作为降低药品微生物负载的唯一途径。药品生产企业应制定灭菌过程控制文件,保持每一灭菌批的灭菌过程参数记录,灭菌记录应可追溯到中药产品的每一生产批。
三、辐照装置
应按质量管理体系要求选择和审核辐照单位,以保障研究和生产过程中的药品质量。中药生产企业应要求辐照单位提供包括辐照产品名称、批号、辐照目的、辐照日期、产品装载模式、辐照装置设定的运行参数、常规剂量计的分布位置和数量、最小吸收剂量、最大吸收剂量、整体平均剂量、剂量不均匀度等在内的辐照记录。
适用于中药辐照的辐射源有:(1)60Co等放射性核素产生的γ射线;(2)电子加速器产生的能量低于5MeV的X射线;(3)电子加速器产生的能量低于10MeV的电子束。可采用静态辐照、动态辐照(包括动态步进辐照及产品流动辐照)等辐照方式。
四、辐照剂量
应分析产品特征,综合考虑处方组成、所含成份类别、微生物负载及抗性等情况,以及国内外的研究报道和实际生产中积累的数据,全面分析和评估辐照对药用物质基础、药物安全性和有效性的影响,确定拟采用的最大总体平均辐照剂量。
建议尽可能采用低剂量辐照灭菌,中药最大总体平均辐照剂量原则上不超过10kGy。紫菀、锦灯笼、乳香、天竺黄、补骨脂等药材、饮片、药粉,以及含有前述一种以上或多种原料的中药半成品原粉建议辐照剂量不超过3kGy。
龙胆、秦艽药材、饮片、药粉及含有龙胆、秦艽的半成品原粉不得辐照。
五、辐照检测
中药生产企业可根据品种的特点建立相应的辐照检测方法,对经辐照的原辅料、半成品进行辐照检测。对含有药材原粉的原料及半成品,若经过1kGy或以上剂量辐照的鉴别,可参考国家已发布的有关鉴别方法,如光释光鉴别法、热释光鉴别法等。
